Technical Assistance: Children?s records should contain documentation of the name and policy number of the child's medical insurance, if applicable.

Comments:
An unannounced monitoring inspection was conducted on 2/7/24 from 10:00am to 11:10am with the provider who was present with one new assistant. There were seven children in care, ranging in age from eight-months to five-years-old, for a total of 16 points. Children were observed having free play and napping. Three child records, the provider?s record, and one caregiver record were reviewed. The provider has current certification in CPR and First Aid. The provider reports that they do not give medication at this time. The emergency drill log, attendance, and menu were also reviewed. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 2/16/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 8VAC20-800-830-C
Description: Based on review of documentation and interview, the provider did not ensure that documentation was maintained of shelter-in-place drills. Evidence: The emergency drill log did not contain documentation of two shelter-in-place drills for the year.

Plan of Correction: A separate drill form has been printed/practiced.

Standard #: 8VAC20-800-120-B
Description: Based on review of two caregiver records, the provider did not obtain documentation that two or more references as to character and reputation as well as competency were checked before employment for one caregiver. Evidence: The record for Caregiver #2, with an employment date of 1/4/24, did not contain documentation of references.

Plan of Correction: References have been updated and contacted and verified.

Standard #: 8VAC20-800-60-B
Description: Based on review of three child records, the provider did not obtain all required documentation for child records. Evidence: The record for Child C did not contain the address of one emergency contact; or the name, address, and telephone number of a second designated person to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: The contact info has been updated.

Standard #: 8VAC20-800-170-A
Description: Based on review of two caregiver records, the provider did not obtain a current
Report of Tuberculosis (TB) Screening form from each caregiver at the time of hire and prior to coming into contact with children. Evidence: The record for Caregiver #2, with an employment date of 1/4/24, did not contain documentation of a TB test or screening.

Plan of Correction: TB screening has been updated with a new test that is within date needed.
