Technical Assistance:
The following standards were reviewed with the provider:

Standard 580.C -
Daily age-appropriate activities shall include opportunities for vigorous outdoor play daily, depending upon the weather, the ages, and health of the children;

Standard 610.A -
Use of media such as, but not limited to, television, videos, video games, and computer shall be limited to not more than a total of two hours per day.

Comments:
An unannounced monitoring inspection was initiated on 02/09/2024 at 10:10 am and completed on 02/09/2024 at 11:10 am. At the time of entrance there were 2 children in care with 1 caregiver. The ages of children in care counted for 4 points. Children were observed eating snack and playing with toys in the play room.
Records were reviewed for the 2 children in care, 2 caregivers and 1 household member.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-A
Description: Based on record review and interview, the provider failed to ensure that all applicants for employment, employees, and household members shall undergo a background check every five years.

Evidence:
1. The background checks indicated below have not been updated every five years as required:
a. The most recent central registry check for caregiver 1 was conducted on 02/05/2018 and the most recent criminal record check was conducted on 05/31/2018.
b. The most recent central registry check for caregiver 2 was conducted on 03/23/2017 and the most recent criminal record check was conducted on 06/05/2018. The sworn statement was completed on an outdated form that lacks current required information.
c. The most recent criminal record check for household member 1 was conducted on 06/13/2018.
2. The provider confirmed that the the background checks on file were the most current on file.

Plan of Correction: The provider stated that updated background checks will be obtained.

Standard #: 8VAC20-800-30-E
Description: Based on interview, the provider failed to ensure that the home's activities and services are conductive to the welfare of children in care.

Evidence:
The lack of access to the information in children's records was not conductive to the welfare of two children in care when the inspector arrived on 02/09/2024.
1. The enrollment records for two children in care were not available to caregiver 2 during the provider's absence from the home.
a. Caregiver 2 was alone with 2 children when the inspector arrived at 10:10 am. Caregiver 2 stated she did not have access to the children's enrollment records nor did she have access to any emergency information with regard to parent contact and emergency contacts.
b. The provider confirmed that she had not left caregiver 2 with access to the children's records while she was away from the home.

Plan of Correction: The provider stated that records will be accessible to all caregivers going forward.

Standard #: 8VAC20-800-150-D-1
Description: Based on record review and interview, the provider failed to ensure that an assistant who is left alone with children in care shall have current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.

Evidence:
1. Caregiver 2, who was caring for two children during the provider's absence, lacked current certification in CPR.
a. The CPR certification on file expired 05/2021.
2. Caregiver 2 verified that she had not obtained current CPR certification.

Plan of Correction: The provider stated that caregiver 2 will obtain current CPR and First Aid certification if she will be left alone with children in the future.

Standard #: 8VAC20-800-150-D-2
Description: Based on record review and interview, the provider failed to ensure that an assistant who is left alone with children in care shall have current certification in first aid.

Evidence:
1. Caregiver 2, who was caring for two children during the provider's absence, lacked current certification in first aid.
a. The first aid certification on file expired 05/2021.
2. Caregiver 2 verified that she had not obtained current first aid certification.

Plan of Correction: The provider stated that caregiver 2 will obtain current CPR and First Aid certification if she will be left alone with children in the future.

Standard #: 8VAC20-800-180-A
Description: Based on record review and interview, the provider failed to obtain for each caregiver a current TB screening every two years from the date of the first screening.

Evidence:
1. The most recent TB screening on file for caregiver 2 was completed on 03/04/2019.
a. Caregiver 2 confirmed that an updated TB screening had not been obtained.

Plan of Correction: The provider stated that caregiver 2 will obtain an updated TB screening.

Standard #: 8VAC20-800-280
Description: Based on review, the provider failed to ensure that potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored away from food in areas inaccessible to children.

Evidence:
1. A spray bottle of a cleaning agent was observed on the table located on the backyard deck.
a. This deck is accessible to children as this is the path used for entry to the backyard play area.

Plan of Correction: The cleaning agent was put away during the inspection.
