Comments:
A monitoring inspection was conducted on 02/06/2024 between the hours of approximately 12:15 pm and 1:15 pm. There were 7 children present, ranging in ages from 7 months to 3 years (20 points), with 2 caregivers supervising. Children were observed while watching videos and playing with toys. Caregivers were observed during diaper changes, bottle feeding and helping the children transition to nap time.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. There were no medication on site today. A total of 2 child records, 2 staff and 2 household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 22.1-289.035-B-2
Description: Based on record review, the provider did not ensure to obtain a fingerprint-based criminal history check determination letter prior to first day of employment for one caregiver.
Evidence:
1) On the date of inspection, the record for Caregiver #2 (start date 02/02/2024) did not contain the results of a fingerprint criminal history check. The provider stated that Caregiver #2 had their appointment on 02/01/2024 and were waiting on the results.

Plan of Correction: The Provider submitted the criminal history determination letter for Caregiver #2 to the Licensing Inspector on 02/08/24. Moving forward, the Provider shall ensure to obtain a determination letter before first day of employment.

Standard #: 8VAC20-800-170-B
Description: Based on record review, the provider did not ensure that documentation of negative tuberculosis (Tb) screening for each caregiver met all requirements.
Evidence:
1) The documentation shall have been completed within the last 30 calendar days of the date of employment. The Tb screening form in Caregiver's #2 (start date 02/02/2024) record was dated 11/16/2023.

Plan of Correction: Moving forward, the Provider shall ensure the screening have been completed within the last 30 days of employment.
