A subsidy health and safety (SHSI) inspection was conducted on-site February 13, 2024. There were 6 children present with 2 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 8VAC20-790-250-F Description: Based on a review of caregiver records and interview, the vendor failed to ensure that each caregiver complete the Virginia Health and Safety Update training annually.
Evidence: 1. The record of caregiver #1 did not contain documentation of the 2023 Health and Safety update. 2. The record of caregiver #2 did not contain documentation of the 2023 Health and Safety update. Caregiver #2 completed the initial 10 hour training on 10/7/22. 3. Standard #: 8VAC20-790-430-B Description: Based on a review of drill records and interview, the vendor failed to maintain a record of the dates of the practice drills for one year.
