Comments:

An unannounced monitoring inspection was conducted on-site February 13, 2024 from 12:55pm to 1:42pm. The provider was available during the inspection. There were 5 children present, ranging in ages from 2 years to 4 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description:	Based on a review of children's records, the provider failed to obtain documentation that a child had been adequately immunized before the child attended the family day home. Evidence: The record of child #2, enrolled 01/03/24, contained documentation of immunizations dated 01/25/24.

Description:	Based on a review of records and interview, the provider failed to ensure to obtain a tuberculosis screening for each caregiver every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence: The record of caregiver #2 contained documentation of the most recent tuberculosis screening dated 02/02/22.
