Comments:
An unannounced monitoring inspection was conducted on February 15, 2024 between the hours of 10:00 am through 11:15am. There were a total of five children (10 points) observed with two caregivers. A total of five children's files and three staff/household member's files were reviewed. The children were engaged in the following: toy play, puzzles, drawing, electronics and other organized activities. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 8VAC20-800-700-A
Description: Based on medication review, a provider did not obtain written authorization from a parent and physician to administer a long term medication for a child in care.
Evidence:
1. Child #1 has epinephrine on site. The provider did not obtain written authorization from the parent and physician to administer the long term medication.

Plan of Correction: The provider will obtain written authorization from the child's parent and physician for the epinephrine. Authorization for epipen was obtained but it expired in August 2023. The authorization for Benadryl had epi pen on it too. Image attached below. I already sent parents a message for new epipen and authorization. They told me earlier he is ok with egg but will still confirm if he needs a new epipen.

Description: Based on documentation review and interview, documentation was not maintained of emergency evacuation drills.
Evidence:
1. Documentation of the January evacuation drill was not available at the time of the inspection.
2. Caregiver #1 confirmed that the evacuation drill was conducted, but documentation was not available at the time of the inspection.

Plan of Correction: The provider will complete the documentation for the January evacuation drill.

Standard #: 8VAC20-800-60-B
Description: Repeat Violation

Based on record review, children's records did not contain required information.
Evidence:
1. Child #1's record (Start date-11/8/2021) did not contain documentation of the review of the child's emergency contact information.
2. Child #2's record (Start date-12/13/2023) did not contain documentation of the name, address and phone number of a second emergency contact.
3. Child #3's record (Start date-01/30/2023) did not contain documentation of the review of the child's emergency contact information.
4. Child #4's record (Start date-01/18/2022) did not contain documentation of the review of the child's emergency contact information.

Plan of Correction: The provider will have the parents to review the emergency contact information and document the record for the annual review. The provider will add the name, address, and phone number of the a second emergency contact to the record.

Standard #: 8VAC20-800-70-A
Description: Based on record review, the parents were not provided in writing Provisions of the family day home's emergency preparedness and response plan before the child's first day of attendance.
Evidence:
1. Child #2's record (Start date-12/13/2023) did not contain documentation of Provisions of the family day home's emergency preparedness and response plan.

Plan of Correction: The provider will give the family the Provisions of the Emergency Preparedness and response Plan to the family and have them to sign it. I did give parents Provisions of the emergency preparedness and response plan. My email dated Dec 13, 2023 has that attached with all the other needed Forms. I can forward my email to you for verification. The parent didn?t return it. I will ask them for it.

Standard #: 8VAC20-800-80-A
Description: Based on record review and interview the provider did not obtain verification of the identity and age of child within seven business days of the child's first day of attendance.
Evidence:
1. Child #2's record (Start date-12/13/2023) did not have documentation of verification of the child's identity and age.
2. Caregiver #1 confirmed that verification of identity and age was not obtained and documented.

Plan of Correction: The provider will get verification of the child's identity and age and document the record.

Standard #: 8VAC20-800-680-A
Description: Based on observation, the caregiver did not wash their hands with liquid soap and warm running water after coming in contact with body fluids.
Evidence:
1. Caregiver #1 wiped a child's nose and did not wash their hands with liquid soap and warm running water.

Plan of Correction: The provider will ensure to wash their hands after coming in contact with body fluids. I definitely wash hands if they get in contact with body fluids but this time I was busy and used a thick padding of tissue so no fluid touched my fingers.
