Technical Assistance:
Care plans and a DOE model form to list food allergies/preferences/sensitivities was sent by email to the provider.

Comments:
A Subsidy Health and Safety inspection was conducted on 2/16/2024. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements are identified on the violation notice. Description: Based on vendor statement, no written allergy list was available.
Evidence:
No list with children's sensitivities, allergies and preferences was dated and posted in a confidential manner.

Plan of Correction: A list with food allergies, sensitivities and preferences will be created and updated as needed.

Standard #: 8VAC20-790-190-B-12
Description: Based on record review, A written care plan was not available for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction;

Evidence:
Child #1 had a documented food allergy on file and had no written care plan available for review.

Plan of Correction: A care plan, signed by the physician, will be requested from the parent.
