Technical Assistance:
When the substitute provider is assuming the provider role they should have access to paperwork and complete a substitute provider log of hours worked in the provider role.

Comments:
An unannounced renewal inspection was conducted today from 11:10am-12:00pm. There were 8 children directly supervised by 2 the substitute and assistant. The home, 3 caregiver records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed doing a number worksheet, playing with dinosaurs and washing hands. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-800-50-A
Description: Based on observation the family day home failed to keep a written record of children in attendance each day.
Evidence:
At the beginning of the inspection eight children were present in care. Six children were recorded as present on on the attendance record.

Plan of Correction: - Attendance record sheet was corrected.

Provider and assistants will filled the attendance form right after kid arriving.

Standard #: 8VAC20-800-60-B
Description: Based on record review, children's records did not contain all required information.
Child 4's record contained the name, address, and phone number of one of the two required emergency contacts.

Plan of Correction: Emergency contact form was sent back to the parents and parents provided all the required information.

Daycare provider will check the forms and information parents provided.
