Comments:
An unannounced monitoring inspection was conducted on 02/08/24 from 8:30 a.m. to 9:47 a.m. with the provider. There were four children in care, ranging in age from three to five-years-old, for a total of seven points. Children were observed building with blocks and playing in home living. Six child records, one staff record, and one household member record were reviewed. The provider has current certification in CPR and First Aid. No medications were on site and the provider understands that they must obtain certification in Medication Administration Training (MAT) in order to give medication. Breakfast served was Cereal, apples, banana and milk. The first aid kit, flashlight, weather band radio, and fire extinguisher were observed. The emergency drill log, attendance, and menu were also reviewed. Areas of non-compliance are identified in the Violation Notice. Standard #: 22.1-289.036-B-2
Description: Based on record review and interview, the provider did not obtain a fingerprint-based background check report every five years from the date of the last check.

Evidence: The record for household member # 1 had a fingerprint-based background check dated 01/03/19.

Plan of Correction: Fingerprint was done on 01/31/24, the results are taking longer.

Standard #: 8VAC20-800-920
Description: Repeat Violation

Based on observation and interview with the provider, the provider did not have documentation of a menu for the current week.

Evidence: The menu for the current week was not posted, an interview with the provider revealed that the menu had not been provided to the parents for the week.

Plan of Correction: Posted

Standard #: 8VAC20-800-100-A
Description: Based on review of six child records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence: The record for Child #1, with a start date of 10/09/23, and Child #5, with a start date of 01/02/24, did not contain documentation of a physical.

Standard #: 8VAC20-800-90-A
Description: Based on review of child records and interview with the provider, the provider did not obtain documentation that children had been adequately immunized before attending the family day home.

Evidence:
1. The record for Child #1, with a start date of 10/09/23, contained documentation of immunizations dated 11/01/23.
2. The record for Child #5, with a start date of 01/02/24, did not contain documentation of immunizations at the time of inspection.

Plan of Correction: Record received from parent.
