Technical Assistance:
An Information to Parents updated model form will be emailed to the provider as a resource.

Additional in person and online trainings will be sent to the provider on an ongoing basis.

Comments:
An unannounced Monitoring inspection was conducted today. A total of 12 children (33pts) with the Provider and 1 Assistant were present. The children were seated at the table eating lunch. 3 children had completed their lunch and were playing on the floor. Lunch today was chicken nuggets and curly fries. Staff were observed sitting with the children encouraging them to eat and offering second helpings. Handwashing and bathroom procedures followed lunch and the children gathered on the floor to watch a program before nap. The crib room, cot room, diapering area and bathroom were observed. Positive interaction between the children and staff were observed. The younger children were engaged in independent activities while the older children engaged with each other. Some were observed looking at books and playing with toys on shelves that were accessible to them. A total of 6 children's files and all staff and household files were reviewed. The previous violations from the last inspection were reviewed, corrections were verified. Some violations were found today. A corrective action for each was discussed. Standard #: 8VAC20-800-730-D
Description: Based on observation of the family day home, medication was kept in an unlocked place.
Evidence:
A medication was found on a shelf above the diapering area. This area was unlocked.

Plan of Correction: This medication will be returned to the parent. It is no longer needed.

Standard #: 8VAC20-800-750-A
Description: Based on review of topical ointments onsite, some ointments were not properly labeled.
Evidence:
3 topical ointments were located in bins above the diapering area. The ointments were not labeled with a child's name.

Plan of Correction: These products are no longer used and will be disposed of.

Standard #: 8VAC20-800-70-A
Description: Based on child record review, information to parents was not provided to parents.
Evidence:
2 out of 5 child records reviewed did not contain the second page of the Information to Parents model form. The second page requires a parent signature of receipt of the information.

Plan of Correction: The second page will be provided to both parents and signatures will be obtained.

Standard #: 8VAC20-800-210-A
Description: Based on staff record review, one staff did not obtain a minimum of 16 hours of annual training.
Evidence:
Staff #2 received a total of 6 out of the required 16 hours of annual training during the past 12 months.

Plan of Correction: The additional training will be completed. Documentation of this training will be maintained in their file.
