Comments:
An unannounced monitoring inspection was conducted today from 10:00am-10:45am. There were 12 children directly supervised by 2 caregivers. The home, 3 caregiver records, 6 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-800-830-A
Description: Based on record review, evacuation procedures were not practiced monthly.
Evidence:
The family day home did not practice an evacuation drill in January, 2024.

Plan of Correction: Evacuation procedures have been updated.

Standard #: 8VAC20-800-120-C
Description: Based on record review, the substitute provider's record did not contain documentation of the time of arrivals and departures as required by 8VAC20-800-140 D.
Evidence:
At the beginning of the inspection, Caregiver 2 was acting as a substitute provider. The time sheet was not completed for the times they were in the role of provider.

Plan of Correction: Timesheet has been updated accordingly.

Standard #: 8VAC20-800-50-A
Description: Based on record review, the family day home failed to keep a written record of children in attendance each day.
Evidence:
ON the date of inspection, twelve children were present in the family day home. Eleven children were marked as present on the attendance.

Plan of Correction: Attendance sheet has been updated.

Description: Based on record review, children's records did not contain all required information.
Evidence:
Child 1, 2, and 5's record have not been reviewed and updated by the parent annually.

Plan of Correction: Child's records will be reviewed and updated.

Description: Based on record review, parents were not provided all required information in writing prior to the child's first date of attendance.
Evidence:
1. Child 6's record contained a signed copy of the provider's emergency plan that did not contain the location of the evacuation location.
2. Child 6's record contained a signed copy of the provider's medication policy that did not have a medication policy checked.

Plan of Correction: Child's records will be updated accordingly.

Description: Based on observation, caregivers could not communicate in English as necessary.
Evidence:
Caregiver 3 could not provide answers in English to basic questions posed during the inspection like "how many children are there today?".

Plan of Correction: Caregiver 3 will start online classes.

Description: Based on observation, child resistant protective covers were not installed on all outlets.
Evidence:
One outlet in the bathroom used by children did not have an outlet cover.

Plan of Correction: Protective covers have been installed.
