Comments:
A monitoring inspection was conducted on Tuesday, February 27, 2024, with the provider. There were 8 children present with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records, 2 staff records, and 1 household members record was reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: (Repeat Violation) Based on review of five (5) child records, the provider did not ensure that all children's records contained all the required information.

Evidence:
1. The record for child #1, did not contain the address for second emergency contact, and did not have documentation of the child?s medical insurance name and policy number. Child # 1?s enrollment date was 4/20/2023.
2. The record for child #2, did not have documentation of the child?s medical insurance name and policy number. Child #2?s enrollment date was 12/27/2023.
3. The record for child #5, did not have documentation of the child?s medical insurance name and policy number. Child # 5?s enrollment date was 8/14/2023.
