Comments:

An unannounced monitoring inspection was conducted today from 12:19pm- 2:15pm. There were 5 children directly supervised by 1 caregiver. The home, 1 caregiver records, 2 children?s records, 1 household member?s records, evacuation drills, and emergency supplies were inspected. Children were observed looking at books. Diapering and handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice. Standard #:	22.1-289.036-A
Description:	Based on record review, the provider did not obtain the results of a search of the Central Registry for 1 of 2 caregiver/household member records every 5 years as required. Evidence: The results of the search of the Central Registry for Household Member #1 were dated 09/18/2018.
Plan of Correction:	A Central Registry Search request will be submitted within 10 business days.
Standard #:	8VAC20-800-780-B
Description:	Based on record review, the provider did not review annually with the parent the emergency contact information required to ensure the information is correct, and obtain the parent's signed acknowledgment of the review for 2 children. Evidence: 1. The most recent documented review of the emergency contact information for Child #1 was dated 12/20/2021. 2. The most recent documented review of the emergency contact information for Child #2 was dated 11/18/2020.
Plan of Correction:	The provider will review the information with the parents.

Description:	Based on record review, emergency evacuation procedures were not practiced monthly. Evidence: 1. The most recent evacuation drill was dated 07/06/2023.
Plan of Correction:	Evacuation drills will be completed and documented every month.

Description:	Based on record review, shelter in place procedures were not practiced a minimum of twice per year. Evidence: The most recent shelter in place drill was completed on 11/12/2022.
Plan of Correction:	Shelter in place drills will be practiced twice per year.
Standard #:	8VAC20-800-580-C
Description:	Based on observation and provider interview, the provider did not provide all required daily age-appropriate activities. Evidence: The provider is not providing the children with opportunities for vigorous outdoor play daily. The provider stated she has not been taking the children outside during the day, as the area she typically uses, the backyard of the home, is not available due to the space being utilized for storage.
Plan of Correction:	The provider will clean the area outside designated for children to play.
