Activities, staffing, training, organization of records( model forms) signatures and dates on forms.

Comments:
An unannounced monitoring inspection was conducted today from 11:30am-12:20 pm. There were 11 children directly supervised by 2 staff. The home, staff records, children?s records, household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, washing hands, potty routines and getting ready for naptime. There was an ample supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-800-60-B
Description: Based on record reviewed, 1 of 2 records reviewed did not have all of required information.

Evidence:
Child A- did not have Authorization for Emergency Medical Treatment signed by parent
Child B - did not have emergency contact address

Plan of Correction: I will get parents to sign.

Standard #: 8VAC20-800-180-A
Description: Based on record review, 1 of 2 staff records reviewed the provider did not obtain for each caregiver a current report of Tuberculosis Screening every two years from the date of the last screening.

Evidence: Staff #1's last documented report of Tuberculosis screening is dated7/2/2021.

Plan of Correction: It is her first day back. I will send her to do it.

Standard #: 8VAC20-800-280
Description: Based on observation, substances and supplies were not kept in areas inaccessible to children.

Evidence:
Diaper creams and cleaning supplies were observed located on a low shelf next to the diaper changing table that is accessible to children in care.

Plan of Correction: I will move up high where children cannot reach.
