Technical Assistance:
Discussion of hand washing (both caregiver and child) after diapering.

Discussion of cutting up bananas for children under 2 years of age.

So far, the Provider has accumulated a total of 5.5 hours, out of a total of the 16 hours needed each annual training year. All corresponding certificates were on file and reviewed.

Comments:
This non-mandated monitoring inspection was initiated by licensing staff and concluded on 02/28/2024, between the hours of 10 am to 11:30 am. In addition, this inspection served as a follow-up to the initial inspection originally conducted on 11/08/2023 to observe and/or review any standard that was left undetermined on that date. Today, there were 4 children present (10 points), ranging from toddler to preschool-age, with 1 caregiver supervising. The children were engaged in the following: games, movie time, learning, snack, diapering, hand washing, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 4 children records, 1 staff file, and 2 household records were reviewed.

Information gathered during the inspection determined that there were (3) non-compliances with the) applicable standards. These violations were documented on the Violation Notice issued to the home.

Violations:
Standard #: 8VAC20-800-100-A
Description: Based on review, 4 of 4 children files indicated that documentation of a physical examination was not available for review.

Evidence -
On the date of inspection (02/28/2024), the following children did not have a physical available for review:
1. Child #1 (start date: 10/23/2023); Child #2 (start date: 07/15/2023); Child #3 (start date: 10/31/2023); and Child #4 (start date: 01/08/2024).
2. All listed start dates were more than 30 days from the date of inspection.

Plan of Correction: Documentation of a physical examination will be obtained for each child and placed in their file.

Standard #: 8VAC20-800-60-B
Description: Based on review, 2 of 4 children files indicated that not all of the required information was contained within each child's record.

Evidence -
1. On the date of inspection (02/28/2024), 1 of 2 emergency contacts were provided for Child #1, and Child #2.
2. This same violation was cited previously during an inspection that took place on 11/08/2023.

Plan of Correction: All missing information will be placed on file.

Standard #: 8VAC20-800-90-C
Description: Based on review, 1 of 4 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations (for a child who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection (02/28/2024), the current immunization page for Child #1 was dated - 08/14/2023.
2. On the date of inspection, Child #1 was under the age of 2 years.
3. Child #1 did not have any religious exemption documentation on file at the FDH.

Plan of Correction: An updated immunization page will be obtained and placed on file.
