Comments:

An unannounced renewal inspection was conducted on 3/6/24 from 8:50 a.m. to 10:26 a.m. with the provider. There were 10 children in care, ranging in age from 20 months to five-years-old, for a total of 17 points. Children were observed watching an educational program and exercising. Five child records and four two staff records were reviewed. No medications were on site and the provider understands that they must obtain certification in Medication Administration Training (MAT) in order to give medication. The first aid kit, flashlight, weather band radio, and fire extinguisher were observed. The emergency drill log, attendance, and menu were also reviewed. Standard #:	8VAC20-800-780-B
Description:	Based on review of five children's records, the provider did not ensure that they reviewed with the parent the emergency contact information annually for each child and obtained the parent's signature acknowledging the review. Evidence There was no documentation of an annual review with the parent signature for Child #1, with a start date of 08/01/22; Child #3, with a start date of 7/20/20; and for Child #5, with a start date of 10/1/20.
Plan of Correction:	I will have the parents review the information and sign off
Standard #:	8VAC20-800-100-A
Description:	Repeat Violation Based on a review of five child records and interview with provider, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: The record for Child #1, with a start date of 8/1/22, had a physical dated 1/5/23; Child #2, with a start date of 5/1/23, had a physical dated 11/28/23; Child #5, with a start date of 10/1/20, had a physical dated 3/8/21.
Plan of Correction:	I will make sure that physicals are received within 30 days of enrollment.

Description:	Based on review of five child records and interview with the provider, the provider did not obtain documentation that children had been adequately immunized before attending the family day home. Evidence: The immunizations on record for Child #1, with a start date of 8/1/22, was dated 1/5/23; Child #2, with a start date of 5/1/23, was dated 11/28/23; Child #5, with a start date of 10/1/20 was dated 3/8/21.
Plan of Correction:	I will get the immunization records before the first day of attendance.
Standard #:	8VAC20-800-140-B-3
Description:	Repeat Violation Based on review of four caregiver records, and interview, the provider did not ensure that they maintained current certification in cardiopulmonary resuscitation (CPR). Evidence: The provider's previous CPR certification expired on 10/2/23.
Plan of Correction:	The online portion is complete the skills test will take place on 3/9/24.
