Comments:
An initial inspection was initiated and concluded on 3/6/2024. There were no children present. The inspector reviewed compliance in standards required for Family Day Homes. A total of 1 staff record and background checks for the business entity individual were reviewed. The inspection started at 9:50 am and concluded at 11:05 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-800-A
Description: Based on record review and staff interview the provider did not ensure the family day home shall have a written emergency preparedness and response plan.

Evidence: The provider confirmed they did not have an emergency preparedness and response plan.

Plan of Correction: Established & implemented a emergency preparedness and response plan.

Standard #: 8VAC20-800-940-B
Description: Based on observation and staff interview the provider did not ensure they had an operable thermometer available to monitor refrigerator and freezer compartment temperatures.

Evidence: The provider confirmed there was not an operable thermometer in the refrigerator or freezer.

Plan of Correction: Purchased thermometer for refrigerator & freezer.

Standard #: 8VAC20-800-250-A
Description: Based on observation and staff interview the center did not ensure that hanging items including window blind or curtain cords, appliance cords, and ropes shall be out of reach of children under five years of age.

Evidence: The provider confirmed the tv chord, speaker chord, and phone chord were hanging and within reach of children. The provider also confirmed the chord for an electric candle warmer was within reach of children and the liquid in the warmer was hot.

Plan of Correction: Removed all hanging items and cords. Removed full speaker and phone cord.

Standard #: 8VAC20-800-330-A
Description: Based on observation and staff interview the center did not ensure that they had a landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation.

Evidence: The provider confirmed the landline phone present was not operable.

Plan of Correction: Purchased new landline phone. Had it installed correctly.

Standard #: 8VAC20-800-430-B
Description: Based on observation and staff interview the center did not ensure accordion expansion gates and pressure mounted gates shall not be used as protective barriers at stair openings.

Evidence: The provider confirmed the protective barrier at the bottom of the indoor stair opening, leading to the upstairs of the home, was a pressure mounted gate.

Plan of Correction: Installed latched gate. Removed pressure gate completely.
