Violations:
Standard #: 8VAC20-800-100-A
Description: Based on review of children's files, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence: Child #1 and Child #3 have been in attendance for more than 30 days and do not have a physical examination on file at the home.

Plan of Correction: Provider will obtain a physical examination for all children within 30 days attendance.

Standard #: 8VAC20-800-40-A
Description: Based on observation and confirmed by the provider, the provider did not ensure that the total number of children receiving care at any one time did not exceed the maximum licensed capacity of the home.

Evidence: The licensed capacity of the home is four, but there were five children in care during the time of inspection

Plan of Correction: Provider will ensure they do not exceed the capacity for their family day home.
