Comments:
An unannounced renewal inspection was conducted on March 7, 2024, from 12:45pm-1:45pm. At the time of entrance, seven children(17pts) were in care with two caregivers present. The inspector reviewed six children?s record and three caregiver's records. Children were observed during the designated rest period., playing with toys, and engaged in other group activities. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report. Violations:
Standard #: 8VAC20-800-790
Description: Repeat Violation

Based on observation and interview, the family day home did not have required posted telephone numbers close to the telephone.
Evidence:
1. The family day home did not have 911 or local dial number for police, fire, and emergency medical responder, the responsible person for emergency backup care posted near the telephone.
2. Caregiver #1 confirmed that the only required emergency number posted near the telephone was the poison control number.

Plan of Correction: The provider will post the required numbers close to the telephone.

Description: Based on record review, children's records did not contain required information.
Evidence:
1. Child #'1s, Child #2's, Child #3's, and Child #4's records did not contain documentation of written authorization if the child is taken off the premises of the family day home.
2. Child #1's record (Start date- 08/25/2021) did not have documentation of the review of the child's emergency contact information.
3. Child #3's record did not have documentation of the medical insurance policy number.

Plan of Correction: The provider will have the parents to review the emergency contact information, provide the medical insurance policy number, and will have the parents to complete authorization for off premises trips to the playground.

Description: Repeat Violation

Based on record review, the family day home did not obtain documentation of additional immunizations for a child.
Evidence:
1. Child #1's record (Start date-08/25/2021) did not contain documentation of additional immunizations. Documentation of the last immunization record was dated 10/04/2022.

Plan of Correction: The provider will ask the parents to obtain updated immunization records from the physician and document the record.

Standard #: 8VAC20-800-150-D-1
Description: Based on record review and interview, current certification in cardiopulmonary resuscitation (CPR) was not obtained for all caregivers.
Evidence:
1. Caregiver #2's record (Date of hire-07/09/2023) did not contain documentation of current CPR. Caregiver #2's CPR expired 01/12/2024.
2. Caregiver #1 confirmed that Caregiver #2's CPR was not current and that Caregiver #2 will attend a CPR class on 03/09/2024.

Plan of Correction: The caregiver has a class on 03/09/2024.

Standard #: 8VAC20-800-150-D-2
Description: Based on record review and interview, current certification in first aid was not obtained by all caregivers.
Evidence:
1. Caregiver #2's record (Date of hire-07/09/2023) did not contain documentation of current certification in first aid. Caregiver #2's certification in first aid expired on 01/12/2024.
2. Caregiver #1 confirmed that Caregiver #2's certification in first aid was expired and that they will take a class on 03/09/2024.

Plan of Correction: The caregiver has a class on 03/09/2024.
