Technical Assistance:
Discussed obtaining permission slips for walks/carriage rides and whenever children and caregivers leave caregiver's property.

Comments:
An unannounced monitoring inspection was conducted 3/7/2024 from approximately 11:04am-12:00pm. There were 11 children (28 points) directly supervised by 3 caregivers. Children were observed playing on the floor with blocks, listening to story and free play. There was an assortment of age appropriate materials for children. The home, 3 caregiver records, 3 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289.011-F
Description: Based on observation and interview, the current inspection notice was not posted on the licensed premises.
Evidence:
The current inspection (9/19/2023) was not posted in the home.

Plan of Correction: Today's acknowledgement form posted at end of inspection. Provider will post signed inspection once, completed.

Standard #: 8VAC20-800-730-D
Description: Based on observation and interview, medication for caregivers was not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. A bin of 12 prescription and non-prescription medications, belonging to caregiver #2 and caregiver #3, was not locked.
2. The bin of medication was found in a white dresser drawer in the basement play area, which was unlocked and accessible to children.

Plan of Correction: Bin of medications to be moved to a locked location in the home inaccessible to children today.

Standard #: 8VAC20-800-60-B
Description: REPEAT
Based on record review and interview, child's record did not contain all required information.
Evidence:
Child#1?s record did not have the name, address and telephone number of a second designated persons to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: Provider will request information on a second emergency contact today at pick up.

Standard #: 8VAC20-800-240-A
Description: Based on observation and interview, playground equipment was in an unsafe condition.
Evidence:
A portion of the playground slide/climber had two pieces with splintered wood.

Plan of Correction: Damaged playground equipment to be removed today.

Standard #: 8VAC20-800-320-B
Description: Based on observation and interview, the home did not have child-resistant protective covers installed on all unused electrical outlets and surge protectors accessible to children under five years of age.
Evidence:
ON the lowere level of the home, with children under the age of 5 present, a surge protector had 5 uncovered electrical outlets.

Plan of Correction: Surge protector unplugged and stored away and outlet covered, when not I use.
