Comments:
An unannounced complaint investigation was conducted on March 6, 2024 in regards to a report of forbidden actions. Interviews were conducted and records were reviewed, The preponderance of evidence does support the allegation and the complaint has been deemed valid. Description: Based on interviews, the provider punished a child for toileting accidents.
Evidence:
1. Child #1 had a toileting accident. Caregiver #1 confirmed that they lightly tapped Child #1's bottom for a toileting accident one time. Caregiver #1 confirmed that the child did not cry and Child #1 did not sustain an injury from the light tap.
2. Witness #1 confirmed that they were present at the time of the incident. Witness #1 confirmed that Child #1 did not cry and that Child #1 did not have an injury as a result.

Plan of Correction: The provider has confirmed that they only lightly tapped the child's bottom once as a result of a toileting accident. The child was not injured and did not cry. The provider has not punished the child for toileting accidents since the incident and will never do it again.
