Comments:

A renewal inspection was initiated on 3/8/2024 and concluded on 3/8/2024. There was one child present during this inspection totaling one point, with one caregiver. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records, three staff records and two household members records were reviewed. The inspection started at 1:30 pm and concluded at 2:25 pm. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description:	(REPEAT VIOLATION) Based on record reviews the provider failed to ensure that every applicant for licensure as a family day home, adult living in such family day home shall undergo a background check every five years after issuance of fingerprint background check. Evidence: Staff #1 and HHM #1 have a fingerprint background check that expired 5/16/23 and 5/15/23.

Description:	Based on record reviews the center failed to ensure that new household members submit to a finger print background check by the end of 30th day for new household members. Evidence: HHM #2 moved into the house February 1, 2024. As of 3/8/24 no fingerprint background check has been obtained.

Description:	Based on observation the provider failed to ensure that when person age 18 years or older begins residing in the home a search of central registry be requested by the individual within 30 days of a person beginning to reside in the home. Evidence: HHM #2 began residing in the home February 1, 2024. The central registry request was not made until February 26, 2024. There is no central registry check on file for HHM #2 as of 3/8/24.
