Comments:

An unannounced renewal inspection was conducted on March 5, 2024 from approximately 10:30 am to 12:00 pm. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, emergencies, caregiver training, preventing the spread of disease and nutrition. The provider record, 1 caregiver record, 1 household member record, and 10 children?s records were reviewed. All areas of the home were observed and areas where the children are cared for were inspected. There were 11 children in attendance for a total of 22 points with 2 caregivers supervising The children were observed learning about magnets, reading stories, reviewing the days of the week and colors, and playing outside while the inspector was on the premises. The first aid kit was inspected and found complete. The menu, emergency numbers, and daily schedule were posted. The information gathered during the inspection determined violations with applicable standards or law. Violations were issued. Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today, 03/11/2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Violations:

Standard #:	22.1-289.035-B-4
Description:	Based on record review, the provider failed to obtain a copy of the results of a criminal history record information check and a sex offender registry check from any state in which the individual has resided in the preceding five years. Evidence: The record for caregiver #2, date of employment 02/12/24 indicated that caregiver #2 has lived in another state outside the state of Virginia in the past 5 years. The record for caregiver #2 did not contain a sex offender registry check and the results of a criminal history record information check from that state.
Plan of Correction:	All sex offender registry checks were done immediately. The out of state agency was also contacted to receive the out of state criminal history record. I will ensure that I check the NFF list to ensure that I have all required out of state background checks and sex offender registry checks done prior to being hired.
Standard #:	8VAC20-800-60-B
Description:	Based on review of ten children's records, the provider failed to maintain complete child records for five children. Evidence: The records for child #1, child #2, child #3, child #4, and child #6, did not contain the names of persons other than the custodial parents who are authorized to pick up the child.
Plan of Correction:	The parents of the children who were missing authorized pick ups, completed that section of the form. To ensure that this doesn't happen again I will make sure that all child record forms are filled out in its entirety before accepting new students.
Standard #:	8VAC20-800-170-B
Description:	Based on record review, the provider failed to ensure that each caregiver obtain a tuberculosis screening within the last 30 days prior to employment. Evidence: The record for caregiver #1, date of employment 02/12/24, contained a tuberculosis screening dated 09/22/23.
Plan of Correction:	The employee got an updated TB screening. I will ensure all future employees have a TB test within 30 das prior to employment.
