Technical Assistance:

Discussed the playground equipment and the sand box. Discussed the menu items and the need for more detail.

Comments:

An unannounced renewal inspection was conducted today from 10:13am-11:05am with the provider on site. There were 5 children (10 points) present with 1 caregiver. A sample of 5 children's files and 4 caregiver/household files were reviewed. The children were engaged in the following: playing with playdough and various toys. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and caregiver were positive. The physical space, attendance records, drill record, menu and the Emergency Preparedness Plan were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only. Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website. Description:	Based on review of children's records, the provider did not have the parent review the emergency contact information required in 22VAC40-222-60 B and did not obtain the parent's signed acknowledgement of the annual review. Evidence: 1. Child #1's file (start date 8/21/2021) had the parent review and acknowledge on 8/19/2022. 2. Child #2's file (start date 9/01/2018) had the parent review and acknowledge on 9/01/2022. 3. Child #3's file (start date 2/28/2022) had the parent review and acknowledge on 2/28/2023. 4. Child #5's file (start date 11/04/2019) had the parent review and acknowledge on 11/25/2022.
Plan of Correction:	I will get the parents to review and sign today.

Description:	Based on review of children's records, the provider did not obtain all required information for the child's record. Evidence: 1. Child #3's file (start date 2/28/2022) was missing both parent's work phone numbers and names and addresses of previously attended child day care and schools.
Plan of Correction:	I will ask the parents for the information today.

Description:	Based on review of children's records, the provider did not obtain from the parent within seven business days of the child's first day of attendance, the name and location of previous day care programs and schools the child had attended. Evidence: 1. Child #3's file (start date 2/28/2022) was missing the names and addresses of previously attended child day care and schools.
Plan of Correction:	I will ask the parents for the information.
Standard #:	8VAC20-800-180-A
Description:	Based on review of caregiver and adult household members records, the provider did not obtain documentation updated tuberculosis test or screenings for each household member every two years. Evidence: 1. Household Member #1 had a tuberculosis screening or test dated 10/06/2021.
Plan of Correction:	I will contact the office for the document. The test was done in February 2024.
