An unannounced monitoring inspection was conducted today from 10:45 am-11:40am. There were 4 children directly supervised by 2 staff. The home, 2 staff records, children?s records, evacuation drills, emergency planning, and policies were inspected. There were 2 medications on site today. Children were observed playing freely , counting, singing songs, practicing walking, giving hugs and preparing to eat lunch. Diapering and handwashing procedures were also observed. There was an ample supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289.035-B-4 Description: Based on record review, the provider failed to obtain an Out of State Abuse and Neglect Search for an employee for a state the employee lived in within the past 5 years.
Evidence:
1. Staff #1 did not have a completed search of out an Out of State Central Registry Search or documentation of follow up within 45 days of submitting the request ( Date of Hire 5/2/2022)Plan of Correction: Staff's identification is Md. She has other documents that are Virginia.

Standard #: 8VAC20-800-180-A Description: Based on review of caregiver records, the provider did not obtain a current Report of Tuberculosis Screening form, every two years from the date of the first screening.
Evidence:
Provider had a tuberculosis test that expired 12/2/2023.Plan of Correction: We will get it done.
