Comments:
An unannounced renewal inspection was conducted on March 20, 2024 from approximately 2:40 pm to 4:40 pm. One caregiver was present with nine children at the onset of inspection. Provider and a second assistant arrived during inspection. Children were observed during naptime and pick up by parents. The inspector reviewed compliance in the areas of administration, personnel, household members, physical health of caregivers and household members, physical equipment and environment, care of children, prevention of diseases, and emergencies, nutrition, and background checks. Five child records and four caregiver records were reviewed for completeness. Additional child records were reviewed for date of birth confirmation. Violations were found and are noted on the violation notice.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on review, provider did not ensure 1 of 4 caregivers has a completed central registry search within 30 days of beginning employment.

Evidence: The record for caregiver #4 (start date 5/16/2023) did not have a completed central registry search.

Plan of Correction: During a time period the state was accepting central registry searches without requiring providers to pay the $10 processing fee. During this time period, May 6. 2023 we submitted an application. The state has no record of receiving it and we didn?t make a copy of the request. Since no payment was required we don?t have proof of purchase. Our correction is to make copies of all mailed correspondence and suspense until complete.

Standard #: 8VAC20-800-210-A
Description: Based on review and interview, provider did not ensure 2 of 4 caregivers complete a minimum of 16 hours of training annually.

Evidence: The following was found during record review:
1. The record for caregiver #1 had documentation of completing 8 hours of training in 2023.
2. The record for caregiver #2 had documentation of completing 10 hours of training in 2023.
3. Training hours were confirmed by provider.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 8VAC20-800-570-A
Description: Based on observation and review, provider did not ensure a caregiver does not exceed 16 points without having an additional caregiver present.

Evidence: Caregiver #3 was observed alone with 9 children from 2:40 pm to 3:00 pm. Children ranged in age from 3 years to 5 years for a total of 17 points. Provider and another assistant arrived to the home at 3:00 pm. The records for child #2 - child #10 were reviewed to verify each child's date of birth.

Plan of Correction: Caregiver's had an assistant with the students and assistant #2 normally clocks out at 3:00PM (8-3). Assistant #3 starts at 3:00PM. Assistant #2 left unexpectedly without prior notice to cover the 20 min time frame. Require assistants to show up 15 mins prior to shift change.
