Comments:
An unannounced monitoring inspection was conducted 3/20/2024 from approximately 10:28am-12:50pm. There were 12 children (24 points) directly supervised by 3 caregivers. The home, 3 caregiver records, 4 children?s records, evacuation drills, emergency supplies, medication and policies were inspected. Children were observed in three areas; outdoors free play, indoor area playing with blocks and in another area, eating snack. There was an assortment of age appropriate materials for children. Areas of non-compliance are identified in the Violation Notice.
Amended to remove previously cited violation.

Standard #: 8VAC20-800-760-A-1
Description: Based on observation, the first aid kit did not contain all minimum required materials.
Evidence: First Aid kit did not contain First Aid instructional manual.

Plan of Correction: Provider to purchase First Aid Kit manual 3/20/24.

Standard #: 8VAC20-800-800-B
Description: Based on observation and interview, the provider did not review the emergency plan at least annually or document in writing each review.
Evidence:
1. Emergency preparedness plan dated 2019 did not have review dates.
2. Provider stated they were unaware of needing to review even if no changes or updates were made.

Plan of Correction: Provider to review and document review of emergency preparedness plan 3/20/24 and annually thereafter.

Standard #: 8VAC20-800-80-C
Description: Based on record review, the provider did not document in the child's record, the method of verification of the child's age and identity.
Evidence: 4:4 children's records did not have date or name of person verifying child's identity.

Plan of Correction: Provider will document method, date and person verifying identity for current children, have parents bring proof of identity and update child's file, for current children. Provider will document method, date and person verifying identity for new children.

Standard #: 8VAC20-800-150-D-1
Description: Based on record review, provider's assistants are not currently certified in CPR.
Evidence: Caregivers 2 and 3 have title of Assistant and are left alone with children, only completed CPR online (8/2023 and 3/2023) and did not complete required in person skills testing.

Plan of Correction: Provider will ensure caregivers 2 and 3 obtain CPR certification in the next 30 days (4/20/2024).

Standard #: 8VAC20-800-150-D-2
Description: Based on record review, Assistants are not currently certified in First Aid.
Evidence: Caregivers 2 and 3, whose titles are Assistant, and are left alone with children, only completed online portion of First Aid (8/2023 and 3/2023).

Plan of Correction: Provider will ensure Caregivers 2 and 3 to obtain First Aid certification within 30 days (4/20/24).
