Comments:

A renewal inspection was initiated and concluded on 3/18/2024. There were 4 children, and 2 providers children present with the provider. The children were observed during free play and morning circle time. A total of 4 child records, 1 staff record, and 1 household member record were reviewed. The inspection started at 9:45 am and concluded at 11:05 am. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description:	Based on document review and staff interview the licensee did not ensure emergency evacuation procedures were practiced monthly with all caregivers and children in care during all shifts that children are in care. Evidence: The licensee confirmed they did not have documentation of emergency evacuation drills during the months of October, November, December 2023, and January 2024.
Plan of Correction:	Will be sure to document future drills as required.
Standard #:	8VAC20-800-60-A
Description:	Based on record review and staff interview the licensee did not maintain an up-to-date record at the family day home for each enrolled child. Evidence: The provider confirmed there was no record available for review for child #1.

Standard #:	8VAC20-800-60-B
Description:	Based on record review and staff interview the licensee did not ensure that every child's record contained all of the required information. Evidence: The provider confirmed the following items were missing from the children's records that were reviewed: 1. The record for child #2 (start date 8/28/2023) did not include the employment phone # for one of the parents listed and did not include proof of the child's age and identity. 2. The record for child #3 (start date 8/28/2023) did not include the primary address and phone # for the 2nd parent listed, the employment phone # for the first parent listed, and the employment name, address, and phone # for the 2nd parent listed. 3. The record for child #4 (start date 8/28/2023) did not include the name, address, and phone # for a 2nd emergency contact.
