Description:	Based on record review, parents were not informed in writing of all required information. Evidence: Child 1, 2, 3, and 4's record had incomplete or blank spaces documented for required policies.
Plan of Correction:	All the blank space in the documents is being filled.

Description:	Based on observation, areas inside the house were not maintained in a safe condition. Evidence: The smoke detector in the child care area was making a beeping noise, indicating it was not working.

Plan of Correction:	I will make sure there are no cords near children's access.
