Two topical skin ointments were observed without being labeled with the child's name.

Evidence:
Child 1, 2, 3, and 4's record had incomplete or blank spaces documented for required policies.

Plan of Correction: All the blank space in the documents is being filled.

Description: Based on observation, areas inside the house were not maintained in a safe condition.
Evidence:
The smoke detector in the child care area was making a beeping noise, indicating it was not working.

Plan of Correction: The smoke detector was corrected the same day

Description: Based on observation, hanging items were not kept out of reach of children.
Evidence:
Two small appliance cords were observed hanging in areas where children had access.

Plan of Correction: I will make sure there are no cords near children's access.

Cleaning and disinfecting products were observed in the bathroom and in a lower kitchen cabinet accessible to children.

The crib mattress is a sheet of foam and is not waterproof.

Description: Based on observation, toilet chairs were not empty and cleaned after use.
Evidence:
The toilet chair was observed to contain urine and feces.

Plan of Correction: After using the bathroom it will be cleaned immediately.
