Comments:
An unannounced monitoring inspection was conducted on March 25, 2024 between the hours of 9:15 am through 10:30 am. There were a total of six children (19 points) observed with one caregiver. A total of six children's files and four staff/household member's files were reviewed. The children were engaged in the following: toy play, bottle feeding, and other organized activities. Violations:
Standard #: 8VAC20-800-570-A
Description: Based on observation and interview, the provider did not ensure that a caregiver does not exceed 16 points.
Evidence:
1. On the date of the inspection, six children (ages: 5 months, 11 months, 11 months, 18 months, 2 years, and 2 years) totaling 19 points were in direct care under the supervision of the provider.
2. The maximum number of points permitted under the supervision of one caregiver is 16 points. Two caregivers are needed for the supervision of 19 points.
3. The provider stated that the assistant had a personal doctor's appointment and would be coming to the family day home after the appointment. Although a household member was present that day, they did not meet the requirements to act as an assistant.
4. The inspection started at 9:15am and the assistant arrived to the family day home approximately 10:40am.

Plan of Correction: The provider knows the point system to determine the need for an additional caregiver. Unfortunately, the assistant had a personal appointment that was an emergency and had to be absent from the family day home for a couple of hours. A household member was present, but did not meet the requirements to be an assistant. The provider will consider having a substitute provider in the future if this happens again.
