Comments:

An unannounced monitoring inspection was conducted on March 19, 2024 between the hours of approximately 8:10 a.m. and 9:05 a.m. There were 10 children (22 points) present with the provider and 1 assistant. Children were observed arriving to the home, watching television and coloring. Positive interactions between the caregivers and children were observed. A complete inspection of physical plant, caregiver, household member and children records, attendance, and emergency drill log was completed. 2 caregiver, 1 household member and 3 children's records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	8VAC20-800-750-A
Description:	REPEAT VIOLATION Based on observation, not all requirements were met when topical skin products were used. Evidence: The family day home did not have written parent authorization for a diaper cream used for Child #3.
Plan of Correction:	I inform the parent to sign the permission slip.
Standard #:	8VAC20-800-90-C
Description:	Based on record review, the family day home failed to obtain documentation of additional immunizations every six months for a child under the age of two years. Evidence: Child #1's (age: 19 months) most recent immunization record was dated 05/31/2023.
Plan of Correction:	I inform the parents & they said they will provide it.
Standard #:	8VAC20-800-280
Description:	Based on observation, the family day home did not ensure that potentially poisonous substances were stored in areas inaccessible to children. Evidence: In the bathroom used by the children, disinfectant wipes and an air freshener spray were located in an unlocked cabinet under the sink, accessible to children.
Plan of Correction:	I will inform all the assistants not to keep those items in the bathroom.
