Technical Assistance:
Provided technical assistance on standard:
1) 830A: the emergency evacuation procedures shall be practiced monthly. Two shelter-in-place drills are practiced in addition to the evacuation drills for a total of 14 drills annually.
2) 880: when family day homes provide meals or snacks, they shall follow the most recent, age-appropriate nutritional requirements of the USDA. (lunch meal pattern includes: meat or meat alternative, vegetable, fruit, grains/breads, milk). Recommend using DOE model weekly menu form
3) 920: Menus must be dated, given to parents, list any substituted food and be kept on file for a week.

Comments:
An unannounced monitoring inspection was conducted from 12:15 - 1:15 with the provider. There were six children present, ranging from four months to four years of age, for a total of 17 points. Children were observed preparing for and eating lunch. Lunch served today: spaghetti with tomato sauce, bananas and milk. Two child records and four caregiver/household records were reviewed along with the daily attendance, emergency drill log, required posted information, and menu. The provider completed certification in Medication Administration Training (MAT) on 6/6/2023 so can administer medications to children in care. Description: Based on review of documentation and an interview with the provider, it was determined that the emergency evacuation procedures were not practiced monthly, as required. Evidence: there was no evacuation drill documented for January 2024.

Description: (Repeat Violation) Based on review of documentation, observation and interview, it was determined that the provider did not ensure all children were accurately included on the attendance report. Evidence: On the attendance record for 3/18/2024, three children were marked in attendance however six children were observed in care.

Description: (Repeat violation) Based on review of two child records, it was determined that not all records contained all required information. Evidence: the record for child #1 and child #2 (date of first attendance: 3/4/2024) did not contain proof of the child's age and identity or the parent's signed acknowledgement of the receipt of the information required by 8VAC20-800-70.
