Technical Assistance:
Consultation was given on crib equipment (not in use) to ensure lower base board is bolted to the crib sides, screw holes should be filled/covered and crib sheet should fit tightly in the mattress. Discussed moving diapering area into the bathroom, closer to running water supply to reduce potential spread of contamination. Also discussed moving gate to floor level, to reduce access to kitchen and stairs. Provider was also given website for CPSC.gov to monitor any recalled child items.

Comments:
An announced initial monitoring inspection was conducted 3/26/24 from approximately 9:30am-11:25am. There were 2 children (7 points) directly supervised by 2 caregivers. The home, 2 caregiver records, 2 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and interacting with caregiver. There was an assortment of age appropriate materials for children. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-770-40-C-1-c
Description: Based on record and interview, not all adults living in the home of an applicant for licensure had completed all required background checks.
Evidence:
1. HHM#1 did not have completed sworn statement or affirmation on file.
2. The provider stated they thought sworn statement was only needed for caregivers.

Plan of Correction: Provider will have HHM's sworn statement completed 3/26/24.

Standard #: 8VAC20-800-760-A-1
Description: Based on observation and interview, the first aid kit did not more than 1 triangular bandage.
Evidence:
1.The only first aid kit in the home, only contained 1 triangular bandage.
2. The provider stated they were unaware that more than 1 triangular bandage was required.

Plan of Correction: Provider ordered triangular bandages.

Evidence: The radio in the home was AM/FM and not weather band.

Description: Based on record review and interview, the provider did not obtain references for assistant.
Evidence:
1. No references in caregiver/assistant's file.
2. Provider stated they were unaware reference was needed.

Description: Based on observation and interview, a landline telephone, excluding a cordless or cell phone, was not available during the family day home's hours of operation.
Evidence:
1. The provider's home did not have a landline telephone.
2. The provider stated they were not aware that radio needed to have weather band.

Plan of Correction: The provider will contact Verizon 3/26/24 and have landline installed.
