Technical Assistance:
Organization of paperwork was reviewed again. A sample of how the children's record should be organized was demonstrated with 4 children's records.

Comments:
An unannounced monitoring inspection was conducted today from 10:15am-12:15pm. There were 9 children directly supervised by 3 caregivers The home, 3 caregiver records, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely. There was an assortment of age appropriate materials for children. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-800-830-A
Description: Based on review of emergency procedures, emergency procedures are not being practiced monthly with all caregivers and children in care.
Evidence
An monthly emergency evacuation drill was not conducted in October, November, December or January.

Plan of Correction: I will conduct a fire drill monthly and record it on the form.

Standard #: 8VAC20-800-890
Description: Based on observation, when food is brought from the child's home, it is not clearly labeled to easily identify the owner.
Evidence
When food is brought from home it is not clearly labeled to identify the owner. There were food pouches, a banana and some food containers not labeled.

Plan of Correction: When food is brought from home, I will make sure it is clearly labeled with the child's full name.

Standard #: 8VAC20-800-960-C
Description: Based on observation, a highchair with a safety waist and crotch strap was not fastened according to the manufacturer's instructions.
Evidence
Child # 1 was placed in a highchair that did not have a safety waist and crotch strap.

Plan of Correction: I washed the strap and didn't put it back on. I will make sure all highchairs have their straps and are used.

Description: Based on observation, bottles in the refrigerator were not labeled with the child's full name and the date.
Evidence
Bottles located in the refrigerator were labeled with the chilld's full name and date.

Plan of Correction: All bottles will be labeled with the child's full name and current date.

Standard #: 8VAC20-800-120-B
Description: Based on review of caregiver records, not all records are complete.
Evidence
The records for staff #1 and #2 do not contain documentation of verification of the caregiver's age.

Plan of Correction: I will put a copy of the caregivers age verification in each record.

Standard #: 8VAC20-800-60-A
Description: REPEAT VIOLATION
Based on review of children's records, the provider did not maintain an up-to-date record for each enrolled child.
Evidence
The record for child #1 does not contain emergency contact information, authorization to apply topicals or documentation of information for parents including policies and procedures for the family day home.
The record for child #2 does not contain documentation of information for parents including policies and procedures for the family day home.
The record for child #3 does not contain information for a second emergency contact.

Plan of Correction: Since going over the records with my inspector, I will go through all the records and make sure to get all the missing information and make sure to get the information before the child's first day.

Standard #: 8VAC20-800-90-C
Description: Based on review of caregiver records, the family day home did not obtain documentation of additional immunizations for a child who is under the age of two every six months.
Evidence
Child #2 (under the age of two), does not have documentation of an updated immunization record every six months. Immunization documentation in record is dated 4/12/2023.

Plan of Correction: I will get an updated immunization record and make sure to get an updated record every six months for children under the age of two.

Standard #: 8VAC20-800-180-A
Description: Based on review of caregiver records, the provider did not obtain from each caregiver a current report of tuberculosis (TB) screening form every two years from the date of the first screening.
Evidence
The TB screening form for staff #2 expired on 2/4/2023.

Plan of Correction: A new TB screening will be obtained. I will make sure there is a new TB every two years.
