Comments:

An unannounced monitoring inspection was conducted on 03/27/24 from 10:00 a.m. to 11:00 a.m. with the provider. There were five children in care, ranging in age from six months to five-years-old, for a total of 11 points. Children were observed cutting shapes out of soft dough. Five child records, two staff records, and one household member record were reviewed. The provider has current certification in CPR and First Aid. No medication was on site, and the provider has current certification in Medication Administration Training (MAT). The first aid kit, flashlight, weather band radio, and fire extinguisher were observed. The emergency drill log, attendance, and menu were also reviewed. Areas of non-compliance are identified in the Violation Notice. Description:	Repeat Violation Based on observation and interview, the provider did not post the most recent inspection of the family day home. Evidence: The findings of the most recent inspection of the family day home was not posted; the documentation of the last inspection was stored in a folder.

Description:	Based on observation and interview, the provider did not ensure a thermometer was present to monitor the temperature in the refrigerator or the freezer. Evidence: The refrigerator and freezer being used to store food for the children in care did not contain a thermometer to monitor the temperature.

Description:	Based on a review of 5 files and an interview, it was determined that one child's file did not have a physical within 30 days of attendance. Evidence: The record for Child #5, with a start date of 02/14/22, had a physical dated 01/10/23.

Description:	Based on observation and interview it was determined that children were not always diapered on a nonabsorbent surface that can be cleaned and sanitized. Evidence: The diaper changing pad had a small tear which makes the pad no longer nonabsorbent.
