Technical Assistance:
Reminder to complete emergency evacuation drills monthly, shelter-in place drills twice a year, and lockdown drills once a year. Complete 16 training hours annually.

Comments:
An unannounced monitoring inspection was conducted on March 20, 2024, from 9:49 a.m. to 11:08 a.m. with the provider. There were seven children, ages 2, 2, 4, 4, 4, 5, and 5 years old, were in the care of one caregiver for a total of 12 points. The children were observed playing with age-appropriate toys, painting, and playing with Play-Doh. Four children?s records were reviewed. The provider and three household member?s records were reviewed. There were no medications on site to be reviewed. The provider has current certification in CPR and First Aid. The first aid kit, flashlight, and battery-operated radio were observed. The most recent practice emergency evacuation drill was documented as 2/12/2024. The most recent shelter in place was documented as 6/5/2023.

Description: Based on a review of four children's records, the provider did not obtain the parent's signed acknowledgment of the annual review of the emergency contact information. Evidence: Child A and B's records were missing documentation of an annual review of the emergency contact information.

Standard #: 8VAC20-800-180-A
Description: Based on a review of documentation, the provider failed to obtain a current Report of Tuberculosis Screening form, every two years from the date of the first screening for all adult household members. Evidence: Household member 1 did not have a documentation of a current tuberculosis screening.
