An unannounced monitoring inspection was conducted today from 8:56am-9:45am with the provider on site. There were 10 children (26 points) present with 4 caregivers. A sample of 2 children's files and 4 caregiver/household files were reviewed. The children were engaged in the following: playing with toys, participating in circle time singing songs, playing games and dancing. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and caregivers were positive. The physical space, attendance records, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Standard #: 8VAC20-800-180-A Description: Based on review of caregivers records, the provider did not obtain for each caregiver and adult household member a current report of Tuberculosis screening form every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
1. Caregiver #1 had a tuberculosis screening form dated 9/22/2021.
2. Caregiver #2 had a tuberculosis screening form dated 9/17/2021.Plan of Correction: Set up an appointment to have TB screening.
