Technical Assistance:

Explained the renewal application is online at www.childcare.virginia.gov/providers. Renewal applications can be submitted 60-70 days prior to license expiration (11/23/24).

Comments:

An unannounced monitoring inspection was conducted on April 8, 2024 between 12:50pm and 2:00pm. There were 10 children present with two (2) caregivers supervising within the supervision guidelines. The children were observed sleeping or resting on their cots, being fed, and engaged in a quite activity. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas of administration, required posted documents, physical plant, staffing and supervision, programing, evacuation drill logs, menu, first aid supplies, attendance, child and caregiver records, and emergency procedures. There was no medication on site. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description:	Based on record review and interview, the provider failed to review annually with the parent the emergency contact information to ensure the information is correct. Evidence: 1. The emergency contact information for Child #1 was not reviewed annually. The last documentation was dated. 01//2023. 2. The emergency contact information for Child #2 was not reviewed annually. The last documentation was dated 10/2022. 3. The provider confirmed the records for Child #1 and Child #2 were not reviewed annually.
Plan of Correction:	I will have the parents review and sign the documentation today when they pick up their children.
Standard #:	8VAC20-800-90-C
Description:	Based on record review, the family day home failed to obtain documentation of additional immunization once every six months for children under the age of two years. Evidence: 1. The record for Child #1 (DOB 03/2022) did not have documentation of additional immunization once every six months. The last immunization record on file was dated 9/13/23.
Plan of Correction:	I will tell the parents today that I need the updated immunization record.
