Comments:
A non-mandated monitoring inspection was conducted on April 9, 2024 from 10:55 A.M.-1:30 P.M. There were eight children present, ranging in ages from 10 months to three years of age for a total of 23 points, with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and two caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-710
Description: Based on review of medication, the provider did not ensure that they had completed the additional medication administration training, and the medication was not maintained in the original pharmacy labeled container.
Evidence:
1. An emergency medication, Auvi-Q, did not have a pharmacy label that includes the child's name.
2. An emergency medication, requires additional training to administer AuviQ epinephrine. The additional training was not obtained by the Provider.

Plan of Correction: Mother was notified Tuesday after the inspection. Wednesday she contacted medicine company. They are sending her the prescription label. Additional class will be taken online in addition to holding the MAT certification.

Standard #: 8VAC20-800-100-A
Description: Based on review of children records, the provider failed to obtain a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
1. Child #2's first date of attendance was 02/12/2024. There was not a physical examination in the child's record.
2. The provider verified that the Child #2's record did not contain a physical examination.

Plan of Correction: Mother was notified during dismissal on 04/09/2024. The doctor faxed the physical to the mother. It is now in the record.

Standard #: 8VAC20-800-60-B
Description: Based on review of children records, the provider failed to ensure all required information was in the children records.

Evidence:
1. Four out of four children records did not contain the address of the two designated persons to contact in case of an emergency if the parent cannot be reached.
2. The provider verified that the records did not contain the address of the emergency contacts.

Plan of Correction: Parent will add the physical address of contacts to the medical emergency contact record in the file.

Standard #: 8VAC20-800-510-D
Description: Based on observation, the center failed to ensure that rest mats that are used must have at least an inch of cushioning.

Evidence:
Five out of eight children did not have a rest mat with at least an inch of cushioning. The children had sleep bags that were laid directly on the floor.

Plan of Correction: Mats have ordered and will be delivered 04/19/2024.

Standard #: 8VAC20-800-680-B
Description: Based on observation, the provider failed to ensure that children were washing with soap and running water before eating.

Evidence:
Two children were in play equipment for infants before lunch. Caregiver #2 took the children out of the equipment and placed in highchairs for lunch. The children's hands were not washed before they began lunch.

Plan of Correction: Assistant has been properly trained since the inspection.
