A monitoring inspection was conducted on April 3, 2024 to follow up on previous violations. The family day home was issued a Notice of Intent to Revoke License on January 25, 2024. There were 9 children (totaling 23 points) directly supervised by 3 staff. The physical plant, 1 staff's record, 3 child's record, and evacuation drills were inspected. Children were observed participating in tummy time, resting and outdoor play. There was an assortment of age-appropriate materials for the children in care.

Standard #: 8VAC20-800-100-A Complaint related: No Description: Based on review children's records, 1out of 3 records, the center did not have documentation of a physical in each child's file prior to or within 30 days of first attendance.
Evidence:
1. Child 1 (date of enrollement 12/0/2020) did not have a physical examination in the record upon review during the inspection.Plan of Correction: ask parents to send it again. Because they bring it on the registration day but I think I lost it when I renew all the foldes. sorry
