Comments:
An unannounced renewal inspection was conducted on 4/11/24 from 8:50am to 10:30am with the provider who was present with two assistants and nine children. The children ranged in age from 11-months to three-years-old, for a total of 24 points. Older children were observed playing on the playground and one infant was observed napping and waking from nap. Four child records, three caregiver records, two household member records, and one pet record were reviewed. The provider and both assistants have current certification in CPR and First Aid. The provider understands that they must obtain re-certification in Medication Administration Training (MAT) in order to give medication. The emergency drill log, attendance, and menu were also reviewed. Description: Based on review of documentation and interview, the provider did not ensure that records of emergency evacuation and shelter-in-place drills were maintained for one year. Evidence: The provider could not locate the drill log for 2023 in order to show documentation of two shelter-in-place drills conducted.

Description: Based on review of four child records, the provider did not obtain documentation of a physical examination by a physician for each child within 30 days of attendance. Evidence: The physical on record for Child D, with a start date of 5/3/22, was dated 8/17/22.

Evidence: The most recent TB screening on record for Caregiver B was dated 9/2/21. The most recent TB screening on record for Household Member A was dated 10/16/21.

Description: Based on review of three caregiver records, the provider did not ensure that all caregivers obtained a minimum of 16 hours of training annually in areas relevant to their job responsibilities.
