Comments:
An unannounced renewal inspection was conducted on 4/11/24 from 10:40am to 11:45am with the provider who was present with one assistant and 12 children. The children ranged in age from eight-months to four-years-old, for a total of 28 points. Children were observed playing outside, coming inside and washing hands, coloring, and eating lunch. Four child records, two caregiver records, and one pet record were reviewed. The provider has current certification in CPR and First Aid. The provider does not give medication at this time and understands that they must obtain certification in Medication Administration Training (MAT) in order to give medication. Lunch served was fish sticks, corn, mandarin oranges, macaroni and cheese, and milk. The emergency drill log, attendance, and menu were reviewed. The carbon monoxide detector was observed. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 4/19/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-800-100-A
Description: Based on review of four child records, the provider did not obtain documentation of a physical examination by a physician for each child within 30 days of attendance. Evidence: There was no physical on record for Child A, with a start date of 2/15/24.

Plan of Correction: The parent had it sent from the doctor on 4/12/24. Moving forward, we will make sure we receive physicals within 30 days of children starting.

Standard #: 8VAC20-800-90-A
Description: Based on review of four child records, the provider did not obtain documentation that each child had been adequately immunized before attending the family day home. Evidence: There were no immunizations on record for Child A, with a start date of 2/15/24.

Plan of Correction: The parent had it sent from the doctor on 4/12/24. Moving forward, we will make sure we receive immunizations prior to children starting.
