Comments:
An unannounced monitoring inspection was conducted on April 12, 2024, from 9:46 a.m. to 10:44 a.m. with the provider. There were four children, ages 18 months, 2, 3, and 4 years old, were in the care of one caregiver for a total of 9 points. The children were observed playing with age-appropriate toys in the indoor play area. Three children?s records were reviewed. The provider and two household member?s records were reviewed and in compliance. There were no medications on site to be reviewed. The provider has current certification in CPR and First Aid.

Description: Based on a review of documentation, the provider failed to ensure the emergency contact information was reviewed annually and obtain the parent's signed acknowledgment of the review. Evidence: The most recent acknowledgement of the emergency contact information for Child A and B were dated 12/26/2022.

Plan of Correction: The provider will have the parent's review the emergency contact information and the records will be updated with the acknowledgement.
