Comments:
An unannounced monitoring inspection was conducted from 10:25-11:25 am with the provider. There were four children present, including one resident child, ranging from two to three years of age, for a total of eight points. Children were observed playing with a variety of age-appropriate toys in Montessori style centers. Three child records and two caregiver/household member records were reviewed in addition to the written daily attendance, menu, emergency drill log, two non-prescription medications and authorization forms and the required posted information. The provider has current certification in Medication Administration Training (MAT) and can administer medication to children in care. If you have further questions about this inspection please contact Donna Liberman at 540-359-5244 or Donna.Liberman@doe.virginia.gov.

Violations:
Standard #: 8VAC20-800-730-D
Description: Based on observation, not all medication was kept in a locked place, using a safe locking method that prevents access by children. Evidence: medication was observed in a container that did not have a locking mechanism.

Plan of Correction: Will obtain a lock for the medicine box.

Standard #: 8VAC20-800-60-B
Description: Based on a review of documentation, not all records contained all required information. Evidence: the record for child #1 and child #2 did not contain the name, address and phone number for a second emergency contact.

Plan of Correction: Will obtain a second emergency contact.
