Technical Assistance:
Written information about Sudden Infant Death Syndrome (SIDS) and Sudden unexpected infant death (SUID) was shared with the provider and was encouraged to be shared with families of enrolled children who have infants.

Information regarding the requirement for Complaint Cribs to be available for infants under 12 months old was shared, to include Federal Mandated Crib Standards. Any crib manufactured after 6/28/2011 is required to meet these federal standards. Manufacturer's information is normally printed on the under side of the crib's platform and a on the printed material included with the crib in the packaging.

Today we discussed outdoor play for children.
When or if you decide to utilize the back yard for children's outdoor play, any unsafe / unstable items that could be knocked over or fall on a child must be removed.
The back outdoor stairs have 3 risers and cannot be accessible to children under the age of 2 or older children who are not developmentally ready to climb or descend stairs without supervision.

Comments:
An unannounced monitoring inspection was conducted on 4/16/2024 from 12:15 pm-1:55 pm. At the time of entrance, 3 children were in care with the licensee. The sample size consisted of 5 children?s records and 1 caregiver record. Children were observed sleeping and quietly watching a children's program on a tablet, and later playing freely with toys and socializing. Medication is not administered to children in care. Transportation was not observed. However, the provider does transport children to and from care. The vehicle and child safety seats, and vehicle insurance were viewed as well as written authorization to transport children in a vehicle. Transportation requirements were reviewed. Children walk / stroll to a neighborhood park , weather permitting, for daily outdoor time. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-520-A
Description: Based on observation and interview, the licensee did not ensure that cribs are provided for children from birth through 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot, rest mat, or bed.
Evidence:
Upon arrival, an 11 month old infant (child #1) was observed sleeping in a car seat. in the room with the licensee.
The license stated that child #1's parent requested that child #1not sleep in a crib while in care and that child #1 does not sleep in a crib at home.
The car seat sits in a mobile rolling frame and the licensee had the infant in close proximity and ensures child #1 remains in sight and sound supervision while she is sleeping.
The licensee confirmed that there is not an available crib in the home and has not been since child #1 enrolled 10/23/2023.

Plan of Correction: The licensee stated that she no longer has a crib available because she had opted to no longer enroll infants quite some time back, However, child #1 is a sibling to another child in care and was permitted to enroll.
The licensee spoke with the parent of child #1 during the inspection and advised the parent that child #1 may not return to care unless or until a crib is provided or until the child turns 1 year old and can safely sleep on a cot (in less than 1 month).
All infants under 12 months of age will be provided a crib if they are enrolled in the future.
