Comments:

A follow up monitoring inspection was conducted today between 9:00am and 10:00am. There was 1 child present with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. The child was observed interacting with a caregiver. A total of 1 child record and 2 caregiver records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards and violations were documented on the violation notice issued. Standard #:	22.1-289.011-F
Description:	Based on observation, the license required by the Superintendent was not posted in a conspicuous place on the licensed premises. Evidence: The provider did not post the license and most recent inspection as required.
Plan of Correction:	I just took it down from the board, I will make sure to always have it up.
Standard #:	8VAC20-800-750-A
Description:	Based on observation when topical skin products are used, written parent authorization and labeling with the child?s name of the topical skin products the standard was not met. Evidence: The topical skin product for Child #1 was not labeled with the child?s name and the provider did not have parent authorization for Child #1.
Plan of Correction:	I will have the parent fill out the form and label the topical product.
Standard #:	8VAC20-800-50-A
Description:	Based on observation the standard for keeping a written record of children?s attendance was not met. Evidence: No daily attendance was taken on 4/18/24.
Plan of Correction:	Since I only have one child in care, I did not realize I had to keep a record of daily attendance. Going forward, I will keep record of daily attendance.
Standard #:	8VAC20-800-130
Description:	Based on observation, caregiver is not able to speak, read, and write in English as necessary to carry out assigned job responsibilities. Evidence: Caregiver #1 was not able to communicate with inspector effectively.

Standard #:	8VAC20-800-280
Description:	Based on observation potentially poisonous substances were accessible to children. Evidence: Clorox cleaning wipes were left on the diaper changing table making them accessible to children.
Plan of Correction:	I will make sure all potentially poisonous substances are inaccessible.
