Comments:

A renewal inspection was initiated on 4/15/2024 and concluded on 4/15/2024. There were seven children present with one provider and an assistance. The point total was 22. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records, two staff records and one HHM records were reviewed. The inspection started at 10:05:00 am and concluded at 11:40 am. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #:	8VAC20-800-100-A
Description:	Based on record reviews the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child #5 began care 5/4/23 and there was no physical on file.
Plan of Correction:	The provider contacted the child's parent during the inspection and the physical was provided at pick up.
