Comments:
An unannounced monitoring inspection was conducted today between 11am and 12:05pm. There were 9 children present with the Provider and 3 assistant for a total of 28 points within the supervision guidelines. The children were observed playing outside in the backyard, transitioning inside, washing hands and eating lunch (chicken quesadillas, grapes, beans and milk). Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations:
Standard #: 8VAC20-800-180-A
Description: Based on record review, the provider did not obtain a subsequent tuberculosis (TB) screening every two years for each caregiver.
Evidence:
1. Caregiver #1s most recent TB screening is dated 3/10/2022.
2. Caregiver #3s most recent T'B screening is dated 3/4/2020.

Plan of Correction: They have an appointment on Monday.

Standard #: 8VAC20-800-200-A
Description: Based on record review, the provider did not orientate each caregiver by the end of their first week assuming job responsibilities.
Evidence: Staff #2 and staff #3s caregivers records did not contain documentation of orientation training.

Plan of Correction: I'll do it today and have them sign the form.

Standard #: 8VAC20-800-280
Description: Based on observation, potentially poisonous substances, such as cleaning age and deodorizers, were not stored in areas inaccessible to children.
Evidence: A bottle of bleach cleaner and an air freshener was observed on an open shelf above the toilet in the childrens bathroom.

Plan of Correction: Corrected during the inspection.

Standard #: 8VAC20-800-320-B
Description: Based on observation, child resistant protective covers were not installed on all unused electrical outlets accessible to children.
Evidence: Two unused outlets were observed above the sink in the childrens bathroom without protective covers.
