Comments:
An unannounced monitoring inspection was conducted on 4/22/24 from 9:05am to 10:05am with the provider who was present with two assistants. There were nine children in care, ranging in age from four-months to two-years-old, for a total of 28 points. Children were observed eating a breakfast of cereal, bananas, and milk, then having free play. Five child records, three caregiver records, and one household member record were reviewed. The provider has current certification in CPR and First Aid. The provider does not give medication and understands that they must obtain certification in Medication Administration Training (MAT) in order to give medication in the future. The emergency drill log, attendance, and menu were also reviewed. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 5/1/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 22.1-289.035-A
Description: Based on review of three caregiver records, the provider did not obtain the results of a search of the central registry for a caregiver every five years. Evidence: The most recent central registry results on record for Caregiver #3 were dated 11/13/18.

Plan of Correction: The application submitted to CRS. As soon as we receive the result will provide inspector a copy.

Standard #: 8VAC20-800-960-G
Description: Based on observation, the provider did not ensure that bottles were not heated in a microwave oven. Evidence: On 4/22/24, the Licensing Inspector observed Caregiver #3 warm Child B?s bottle in the microwave.

Plan of Correction: The childgiver who mentioned oriented and won't be repeated.

Standard #: 8VAC20-800-100-A
Description: Based on review of five child records, the provider did not obtain documentation of physical examinations by or under the direction of a physician within 30 days after the first day of attendance. Evidence: The records for Child B, with a start date of 10/16/23; Child C, with a start date of 9/18/23; and Child E, with a start date of 10/11/23, did not contain documentation of a physical.

Plan of Correction: The physical examination for mentioned children have been obtained.

Standard #: 8VAC20-800-90-A
Description: Based on review of five child records, the provider did not obtain documentation that children had been adequately immunized before attending the family day home. Evidence: The records for Child B, with a start date of 10/16/23, and Child E, with a start date of 10/11/23, did not contain documentation of immunizations.

Plan of Correction: The document were missing somehow and we obtain already the necessary document.
