Comments:
An unannounced 60 day follow up monitoring inspection was conducted on April 29, 2024 between the hours of 11:20 am through 12:20 pm. There were a total of one child (4 points) observed with one caregiver. A total of one child's file and two staff/household member's files were reviewed. The children were engaged in the following: toy play. Areas of non-compliance are identified in the violation notice. Please contact me if you have any questions at Karen.dickens@doe.virginia.gov.

Violations:
Standard #: 22.1-289.011-F
Description: Based on observation and interview, the family day home did not post the findings of the most recent inspection in a conspicuous place.
Evidence:
1. The findings of the most recent inspection was not posted in a conspicuous place in the family day home. The provider confirmed that the most recent inspection findings was not printed and posted.

Plan of Correction: The provider will print the most recent inspection report and post it in a conspicuous place.

Standard #: 8VAC20-800-70-A
Description: Based on record review and interview, parents were not provided required information in writing before the child's first day of attendance.
Evidence:
1. Child # 1's record (Start date-04/01/2024) did not contain documentation of whether or not there is liability insurance of at least $100,000 per occurrence and $30,000 aggregate in force on the family day home operation.
2. The provider confirmed that the parents were not provided the declaration of liability insurance before the child's first day of attendance.

Plan of Correction: The provider will give the parents the department's model form for the declaration of liability insurance and have them to sign it. The provider will place the document in the child's record.
